چکیده

A common pitfall in the diagnosis of OA is misinterpretation of the patient's symptoms and signs. Pain is the predominant symptom in patients with OA, and OA pain has typical characteristics. However, pain may arise not only from intra-articular structures but also from periarticular muscle spasm or soft tissue rheumatism. Differentiation of articular pain from periarticular pain is important because the latter often may be managed successfully by local injection of a depot corticosteroid preparation and physical therapy, without systemic medication. In this 3-part article, I review the pathogenesis, diagnosis, and management of OA. The first part (“Defining osteoarthritis: What it is, and what it is not,” The Journal of Musculoskeletal Medicine, September 2010, page 338) offered a contemporary, operational, evidence-based definition of the disease that has evolved from growing knowledge of its cause. In this second part, I discuss the clinical aspects of OA, including diagnosis and the diagnostic pitfalls. The third part, to appear in an upcoming issue of this journal, will provide an overview of current approaches to treatment. THE CLINICAL PICTURE

Ultrastructural Diagnosis of Soft Tissue Tumors: Avoiding Pitfalls

The use of immunohistochemistry and, more recently, molecular methods has seen a sharp reduction of the role of electron microscopy in the diagnosis of soft tissue sarcomas (1,2). Ultrastructural analysis, however, does continue to play a useful role in selected cases. The focus in this presentation is on adult soft tissue tumors; Dr. John Hicks will follow with a presentation on avoiding pitfa...

Diagnostic pitfalls of pilomatricoma on fine needle aspiration cytology

Pilomatricoma is a benign skin adnexal tumour usually seen in the head and neck region of children and young adults. It is underrecognized on cytology, resulting in the overdiagnosis of malignancy. We bring forth a case report of a slow growing nodular swelling in a 10-year-old female child, which was misdiagnosed on Fine Needle Aspiration Cytology (FNAC) as a malignant neoplasm and found to be...

Nodular Fasciitis – Fine Needle Aspiration Cytology Diagnosis and Its Pitfalls, with Review of Literature

Background and Objective: Nodular fasciitis (NF) is a self-limiting, transient neoplasm composed of fibroblasts and myofibroblasts. Since it regresses spontaneously, diagnosis by fine needle aspiration (FNA) cytology plays a major role in its management. Methods: We present a series of 8 cases with either FNA or biopsy diagnosis of NF, and study the major...

Knee osteoarthritis diagnosis, treatment and associated factors of progression: Part II

Diagnosis of knee osteoarthritis can be confirmed based on clinical and/or radiological features. The potential of a progressive disease can be prevented or decreased by earlier recognition and correction of associated factors. Obesity and alignment especially varus malalignment are recognized factors of a progressive disease. Both nonpharmalogical as well as pharmacological modalities of treat...
